Valencia Home Health Care is founded and owned by a compassionate and experienced nurse practitioner to meet the needs of the growing population of individuals, who require intermittent health care services in the comfort of their homes. Valencia Home Health Care was established in 2015 to help patients in advocating their health needs, while caring and dedicating our expertise for their safety and improving their lives.
The principal goal of establishing Valencia Home Health Care in the community is to help accommodate and assist the needs of patients without trained professionals who wish to live in their homes or community. We want to assist them reach their goals and potentials while attending to their physical, emotional, and medical needs.
Our Organization is a full-service Home Health agency providing intermittent care to patients in their homes. Services provided are not limited to: Patient Care, Medication Teaching and Administration, Surgical After-Care, Pain Management, IV-Infusion, Wound Care, Ostomy Care, Tracheostomy Care, PT/INR Testing at home, Diabetes Care and Management, Laboratory Specimen Procurement, X-ray at home, Chronic Disease Management, Integrated Care Management and Coordination of Care with other disciplines, and healthcare services.
